Analyze these perforations

Browsing, I came upon this. Gleaning from what I've learned, I suspect re-perf attempts rather than absent perf splines. Am I off here? Over-reading it?

I would think these result from broken pins on the perforating machine

Bottom perfs do not line up with the top perfs. Left and right sides seem ok.
