Signed by (W.L. Jamison ?) President and Secretary, both signatures with two punch holes. Three additional punch holes to document. Endorsed on reverse. Slight clipped top left corner, 7 punch holes, normal file folds affecting stamps, otherwise light general wear & toning. Fine condition. Base shipping is $3.00 domestic and $4.00 international.
